O'Donnell Hall Facilities Problem Report Form

Welcome to the online Facilities Problem reporting system for O'Donnell Hall. We will evaluate the problem and either take care of it if we can, or put in a workorder to OFS (HVAC, plumbing, electrical, etc.), the police (vandalism, grafitti), ICT (electronic locks, network), or the Janitorial stafff.

Please keep in mind that we have little control over many issues regarding the building (e.g. Heating and Cooling), but will report your issues to the appropriate office. One purpose of this reporting system is to give us a record of what type of problems are occurring.
