Current Distance Education Course Authorization Form

This form was developed by the Office of Distance Education as a way to homogenize the process of getting administrative approval for teaching a Distance Education Course. The form available online in pdf format contains editable fields allowing you to type in your responses directly on your screen.

Please follow these instructions to complete the course authorization form.

Course Authorization

• Go to the course authorization site. A pdf version of the form will pop up. If you do not have Adobe Acrobat Reader in your computer, click on the link to get a free copy of the software.
• Click on the Save icon located on the Acrobat Reader toolbar. Name your file and save it to a convenient place on your computer.
• Fill out the form completely by placing your cursor at the beginning of the first line and typing your response. Use the TAB key to move from line to line. When you are finished, be sure to save your changes.
• Print out the form and route it for approval signatures in the College of Education
  o Head Department approval
  o Assistant Director of Distance Education (Dr. Juanita Mendoza-Hannan)
